Referral Leads — Pre-Fill Submission (Mcard, S&C)

Demo: 10 leads in a table. Click Start submission to create a submission and get a save-and-continue link (with p_id). All logic runs in pre-fill-submission.embed.js; this page only provides the container and config.

Flow: Create submission → modal shows link with p_id → click "Continue to application" to redirect the merchant to the Revenued application.
